Returning Students 2025-2026 School Year
Required form from everyone: (Returning and New) Registration Payment Agreement 1&2, Payment Authorization 4, Child Pick-up 5, Emergency/Health 6, Tech Use 8
The required registration forms for returning students in 1st to 8th Grade are available by clicking below. Please fill out and return to the school office.
We will also need your insurance card and drivers license

New Students and Kindergarten 2025-2026 School YearIf your student is a Kindergartener, or if you are enrolling your student at St. Paul Lutheran for the first time please download, fill out, and return the below forms to the school office. We will also need your birth certificate, immunization record, insurance card and drivers license
These are in addition to the returning student forms needed.
